Ordering Instructions

1. If you using PayPal^® click here. If you are using any other payment method continue to step 2.
2. Open the order form by clicking on the Order Form button below.
3. Fill out the order form. Be sure to include all information requested so that we can process your order in a timely manner.
4. Once the order form is complete, print it out and sign in the space provided.

5. In a sturdy shipping box place the following items.

   1. Your shoe(s)

   2. An envelope containing your order and a photocopy of your prescription.

6. Seal the box securely.

7. Ship the box by the carrier of your choice to the following address:
